HEALTH INFORMATION
To co-ordinate the flow of health information based on the National and Provincial indicators and key performance areas designed to create awareness and reduce health risks through the development of plans, profiling incidences and trends of diseases, Investigate processes and dissemination of information. Advise on status, threats and measures considered during the formulation of strategies with respect to create, maintain and sustain a healthy community.
The District Health Information System (DHIS) being one of the systems used is a National database programme where these required indicators are collected monthly by means of a structured District Health Information Data Flow Policy as directed by the Province. All public and private health facilities, non-governmental Organizations (NGO), Community-based Organizations (CBO), Faith-based organizations (FBO) rendering health services need to adhere to this data flow policy in submission of health data. Monthly Primary Health Care statistical forms are available to complete data for submission to our Health Information Unit. Request can be directed via contact details below.
The Health Information Unit adheres to the principles of the Access to Information Act of November, 2000. To abide to the Act any request for health information by an unofficial person should complete the prescribed form to state their reasons.
